Case Manager

Compensation: $23 Hourly

We are looking for a compassionate Case Manager to support families experiencing homelessness through coordinated case management and housing stabilization services in San Francisco, California. This Long-term Contract position will focus on helping participants overcome barriers to housing, strengthen long-term stability, and access essential community and service-related resources. The role is ideal for someone who can balance direct client support, documentation, outreach, and cross-functional collaboration in a mission-driven non-profit environment.


Responsibilities:

• Manage the intake process for new participants by completing assessments, confirming eligibility, and creating accurate electronic client records.

• Conduct outreach in community settings to engage individuals experiencing homelessness and connect them with available housing and support services.

• Carry an assigned caseload and provide ongoing case management for participants and their families with a focus on housing and employment goals.

• Visit participants in their homes or other approved locations to monitor progress, address concerns, and support housing retention.

• Partner with clients to build individualized service plans, establish practical budgets, and define short- and long-term goals.

• Coordinate referrals to critical resources such as benefits, healthcare, dental care, mental health treatment, substance use services, crisis support, and income assistance.

• Work closely with housing staff and landlords to encourage stable placements and maintain constructive housing relationships.

• Facilitate regular participant meetings to review progress related to financial stability, education, employment, and independent living.

• Maintain complete, confidential case documentation and enter program information into agency databases within required timelines while preparing reports for program needs.

• Contribute to group activities, housing clinics, trainings, and workplace safety practices while supporting overall program operations as needed.

Qualifications:

• Experience providing administrative support, case coordination, or client-facing services in a social services, housing, or non-profit setting.

• Strong ability to manage documentation, data entry, and electronic files with a high level of accuracy and organization.

• Comfortable engaging with clients in person, by phone, and in community-based environments with empathy and respect.

• Ability to handle crisis situations calmly and connect individuals to appropriate services and community resources.

• Effective communication and collaboration skills for working with participants, program staff, landlords, and external service providers.

• Proficiency with general office tasks, including inbound call handling, scheduling, record maintenance, and reception-related duties.

• Bachelor’s degree in social services or a related field is preferred.

• Commitment to confidentiality, clear boundaries, and respectful service delivery within a mission-driven environment.
