Brief Job Description:

We are looking for a Case Manager to coordinate services, support clients, and monitor progress toward program goals.

Responsibilities:

• Conduct client assessments and develop service plans
• Connect clients with community resources and support programs
• Maintain detailed case notes and documentation
• Monitor client progress and follow up on services provided
• Collaborate with internal staff and external partners

Qualifications:

Requirements:

• Experience in case management, social services, or behavioral health preferred
• Strong interpersonal, assessment, and documentation skills
• Ability to manage sensitive information with confidentiality
• Knowledge of community resources and referral processes
• Bachelor’s degree in social work, psychology, human services, or related field preferred
