Case Manager

We are looking for a compassionate and organized Case Manager to support formerly homeless adults in San Francisco, California through a Contract position. This role focuses on helping residents maintain stable housing, access community-based services, and work toward individualized goals that strengthen well-being and independence. The ideal candidate brings strong case coordination skills, sound judgment in crisis situations, and the ability to build trust with tenants facing mental health, substance use, and medical challenges.

Responsibilities:
• Oversee an active portfolio of residents, providing consistent case management support tailored to each individual’s housing stability and personal service goals.
• Meet with tenants at move-in, during the first several months of residency, and on an ongoing basis to assess needs, monitor progress, and address emerging concerns.
• Develop practical service plans with residents and deliver ongoing guidance that promotes housing retention, improved daily functioning, and greater quality of life.
• Help tenants secure and maintain public benefits, stay current with rent obligations, and resolve barriers that may affect continued tenancy.
• Coordinate with property staff during unit reviews, pest control visits, and habitability follow-up to address apartment-related issues impacting residents.
• Connect tenants with employment resources, health and behavioral health providers, and other community programs, while encouraging successful follow-through on referrals.
• Foster a supportive residential environment by organizing tenant meetings, community activities, and collaborative engagement efforts with onsite staff and residents.
• Respond appropriately to urgent tenant situations by using de-escalation techniques, documenting incidents, and involving internal teams or external providers when needed.
• Maintain accurate case records, service documentation, and confidential tenant files in accordance with program standards and reporting expectations.
• Participate in scheduled meetings, recertification activities, check distribution support, and mandated reporting related to suspected abuse or neglect when required.

Qualifications:

• Bachelor’s degree required, or at least 2 years of verifiable experience providing case management services to homeless or formerly homeless individuals in place of a degree.
• At least 1 year of experience working with diverse low-income, homeless, or formerly homeless adult populations.
• Hands-on background in case management, outreach, or supportive housing services.
• Knowledge of San Francisco resources and service networks for homeless and formerly homeless residents, with the ability to make effective referrals.
• Ability to organize, maintain, and update detailed case files for a high-volume caseload.
• Proficiency with Microsoft Office and the ability to prepare reports, correspondence, and other documentation accurately.
• Strong problem-solving, interpersonal, and communication skills, including the ability to remain effective in challenging or crisis-driven situations.
• Experience with documentation, filing, and record maintenance in an administrative or human services setting.

Compensation

$25.00-$25.00 Hourly
