Job Summary:
Essential Functions:
- Provide direct care and supervision to adults residing in the Phoenix Center to ensure safety, stability, and adherence to program expectations.
- Monitor and observe clients’ physical, emotional, and behavioral status throughout shifts, identifying changes in presentation, mood, behavior, or functioning.
- Promptly report changes in client condition, safety concerns, or behavioral observations to the treatment team in accordance with established protocols.
- Support the therapeutic milieu by fostering a calm, respectful, structured, and trauma-informed environment.
- Assist clients with daily routines and scheduled activities, including groups, personal care, leisure activities, and wellness activities as assigned.
- Complete admission and discharge processes, including orientation to the unit, review of rules and expectations, and completion of required documentation.
- Assist with meal preparation, meal service, and cleanup for clients and staff in accordance with health, safety, and sanitation standards.
- Maintain unit cleanliness, organization, and safety, including monitoring common areas as required.
- Support de-escalation efforts using approved techniques and follow safety protocols during behavioral or crisis situations.
- Collaborate with multidisciplinary treatment team members, including clinicians, nurses, peer support specialists, and supervisors, to support coordinated care.
- Accurately and timely complete required documentation, logs, and reports in compliance with RiverValley Behavioral Health policies and regulatory standards.
- Participate in shift handoffs, staff meetings, training, and supervision as required.
- Perform other duties as assigned, as necessary to support program
Qualifications:
Qualifications
- High School diploma or GED
- Valid driver’s license
- Experience working in residential, crisis, healthcare, or direct care settings preferred
- Ability to observe, document, and communicate client status effectively
- Strong interpersonal, teamwork, and problem-solving skills
- Ability to remain calm and professional in high-stress or crisis situations
- Basic computer skills and ability to complete electronic documentation
