Case Manager I

Position Summary

Provide direct support and case coordination services to clients, assisting them with accessing resources and achieving program goals.

Responsibilities

• Conduct client assessments.
• Develop service plans.
• Connect clients with community resources.
• Maintain accurate case notes and documentation.
• Monitor client progress.

Qualifications:

Requirements

• Bachelor's degree or equivalent experience.
• Experience in social services, housing, or workforce development preferred.
• Strong documentation and communication skills.
• Bilingual skills preferred.
