Home Care Intake Coordinator

Compensation: $21 Hourly

We are offering a long-term contract-to-hire employment opportunity for a Home Care Intake Coordinator in La Crosse, WI. This is a hybrid position. You will be working remotely and will eventually need to do onsite client visits once hired on.


Role Overview

The Service/Program Coordinator manages a caseload of Medicaid program participants, coordinating in-home care services while ensuring compliance with state/federal regulations, Managed Care Organization (MCO) requirements, and contract guidelines. This role supports client intake, assessments, care plan updates, service coordination, and ongoing client advocacy.


Key Responsibilities

  • Manage and coordinate a caseload of Medicaid members receiving in-home services
  • Complete client intake, referrals, assessments, and enrollment processes
  • Coordinate in-home care needs and update service plans based on changing client needs
  • Conduct regular client check-ins via phone, video, and in-home visits as required
  • Ensure compliance with Medicaid regulations, MCO guidelines, and company policies
  • Advocate for clients and collaborate with families, providers, stakeholders, and community partners
  • Maintain accurate documentation and utilize web portals/EVV systems
  • Participate in provider/stakeholder meetings, health fairs, and community outreach
  • Provide excellent customer service and support daily operations
  • Handle multiple priorities, complex programs, and special projects independently

Qualifications:

Education:

  • HS Diploma or GED required
  • Bachelor's degree in a healthcare related field preferred

Experience:

  • 1+ year experience working with:
  • Individuals with disabilities
  • Elderly populations
  • Medicaid populations
  • Care coordination/service coordination
  • Case management
  • Community health or social services

Required Skills

  • Strong written and verbal communication
  • Excellent organization and time management skills
  • Ability to manage a caseload independently
  • Problem-solving and decision-making abilities
  • Comfortable making frequent phone calls and conducting virtual meetings
  • Computer proficiency and ability to learn new systems
  • Strong customer service and advocacy mindset
  • Ability to conduct home visits after conversion to permanent status



Compensation

$21.00-$21.00 Hourly
