Job Summary

The Older Adult Clinician plans and provides community integration services and psychotherapy for older adults with severe psychiatric impairment and performs Bio-psychosocial Assessments and Eligibility Determinations for intake into Older Adult Program services.


Responsibilities

  1. Provides comprehensive bio-psychosocial assessments and determines eligibility for Older Adult Program services.
  2. Completes Functional Impairment Assessment and Consumer Certification for Eligibility for CSP Case Management Consumers.
  3. Cooperatively, with the consumer and other appropriate parties, develops and renews when due the individualized service plan (ISP).
  4. Cooperatively, with the consumer, develops Individualized Wellness and Recovery Plan that allows the consumer to manage emerging crisis and to identify resources for maintaining the individual in the community, to the greatest possible degree, during periods of crisis.
  5. Facilitates and coordinates the individual’s treatment planning to assure maximum impact of the services and treatment provided.
  6. Provides individual or group counseling/therapy for individuals who have this service recommended and included in his/her ISP.
  7. Monitors referrals to specialized treatment options as defined in the Individual Service Plan and participates in the interdisciplinary teams of these providers to the degree possible.
  8. Participates in the delivery of treatment options including individual and group Mental Illness Management Services and Case Management Services.
  9. Receives regular supervision from the Older Adult Program Manager
  10. Participates actively in peer and group supervision and administrative team meetings.
  11. Be familiar with productivity and compliance expectations and maintain or exceed productivity goals while meeting all compliance requirements.

Other Expectations

  1. Maintains the consumer’s case record information and documentation in an accurate, timely and professional manner.
  2. Completes any other documentation as required by MFS.
  3. Maintains appropriate auto insurance coverage (as determined by the employee and their insurance company) for use of a vehicle for the purposes of accomplishing the job duties and protecting themselves from action in the event of an accident during the performance of these duties.
  4. Be familiar and in compliance with MFS policies.
  5. Participate in training opportunities required for the position.
  6. Always behave in a professional and ethical manner.
Requirements:

Requirements

  1. Master’s Degree in psychology, social work, psychosocial rehabilitation or related human services field, and is Eligible for NH license and third party reimbursement.
  2. Competent in individual and group psychotherapy.
  3. Familiarity with severe and persistent mental illnesses.
