Department Overview
Our Hospital-Based Internal Medicine Clinic offers complete primary and specialty care to adults aged 18 and older with various health needs, prioritizing inclusive and patient-centered care. The collaborative team consists of over 40 faculty providers, 45 resident physicians, and more than 60 staff members. The clinic sees 175 to 250 patients daily. It also provides non-visit support and follow-up services to 13,948 lives. As part of an academic medical center, the clinic is committed to clinical excellence. It advances medical education by training future internists while serving the community's diverse needs.

Patient Populations:
Serving patients aged 18 and above, primarily from Multnomah, Clackamas, and Washington counties with limited out-of-area exceptions.

Department Key Responsibilities:
Supports complex, high-risk patients in the outpatient setting through advanced care coordination a
- Leading multimodal triage and proactive care coordination, utilizing telephone, electronic messaging, and in-person communication to facilitate timely clinical interventions and reduce avoidable acute care utilization.
- Embedding clinical nursing expertise within ambulatory workflows, supporting real-time decision-making and seamless care transitions across settings.
- Serving as a central point of contact for patients, families, and interdisciplinary team members to promote continuity of care and address emerging clinical concerns in a timely, efficient manner.
- Designing, implementing, and refining ambulatory care models that support safe, scalable, and sustainable nursing practice in alignment with clinic operations and patient population needs.
- Driving improvements in patient outcomes by integrating nursing judgment with patient education, system navigation, and advocacy in a complex outpatient care environment.
Required Qualifications
- Minimum of 6 months of independent RN practice
- BSN Graduates: Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date.
- ADN Graduates: Associate Degree in Nursing from an accredited program 30 days before the start date.
- Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire
- Current, unencumbered Oregon State Registered Nurse License
- Active Washington RN License within 6 months of hire
- BLS from AHA required. New hires will be enrolled and required to complete during orientation.

Preferred Qualifications
- Nursing professional certification in ACRN (HIV/AIDS Certified Registered Nurse) or AACRN (Advanced HIV/AIDS Certified Registered Nurse)
- Experience in an ambulatory care setting, including telephone triage, longitudinal care management of complex patients, and nurse visits
- Experience working in a Primary Care setting
- Working knowledge of Ryan White program requirements and the comprehensive system of care model
Additional Details
Length of Orientation – Experienced Nurse
- External candidates: OHSU & Nursing New Employee Orientation (NEO) for about a week.
- Either Transition to Practice (TTP) Program Specialty Fellowship/Fellowship Learning Pathway or Orientation experience for 3 days-26 weeks depending on the care area. An employment service agreement may apply.
