Would you like to be part of a highly-skilled clinical team including home-based Social Workers focused on supporting individuals in maximizing their health and productivity?

Social Worker Responsibilities


As a Social Worker, you will work independently in your home office setting while being part of a supportive nationwide team providing continuity of care for assigned members across the continuum of care.


  • Provide continuity of care for assigned members across acute and post-acute settings
  • Conduct biopsychosocial assessments to support care planning and identify member needs
  • Collaborate with physicians, case managers and care teams to coordinate services and support care goals
  • Serve as a liaison between members, families and healthcare providers to ensure clear communication
  • Support members with behavioral and mental health needs including connecting them to appropriate care and crisis resources when necessary
  • Provide telephonic psychosocial support and assist members in understanding and managing their health conditions
  • Connect members to community resources, assist with benefits and insurance navigation and support access to services
  • Triage referrals and support the referral process ensuring members are directed to appropriate care and resources


Social Worker Qualifications


  • Bachelor's (BSW) required, Master's (MSW) preferred
  • Valid and Current State license required
  • 3+ years related social work experience with at least 1 year of experience working with mental/behavioral health conditions required
  • Qualified Mental Health Professional (QMHP), Mental Health Technician Certification (MHTC), Certified Clinical Mental Health Counselor (CCMHC) preferred
  • Working knowledge of assessing and addressing the specific needs of the targeted population
  • Identification, diagnosis or treatment of Post-Traumatic Stress Disorder (PTSD) preferred
  • Effective interpersonal, communication and problem-solving skills
  • Ability to maintain a calm, supportive attitude during crisis or potential crises
  • Attention to detail, timetables, and commitment to completing tasks
  • Experience with Microsoft Windows and computer savvy
  • Must be well organized, efficient, and able to work independently and within a team
  • Responsible for having reliable High-Speed Cable or Fiber Optic Internet service and an Internet Router in an established home office
  • Must have Excellent Communication Skills via Phone, email, Text, Verbal and Documentation Skills and provide 24-hour follow-up to all communication
  • U.S. Citizenship
  • Must be able to successfully pass a preliminary credit and background check prior to hiring and training

Sedgwick Government Solutions provides a reasonable range of compensation for roles that may be hired in jurisdictions requiring pay transparency in job postings. Actual compensation is influenced by a wide range of factors including but not limited to skill set, level of experience, and cost of a specific location. For the jurisdiction noted in this job posting only, the range of starting pay for this role is ($72,000-$77,000). A comprehensive benefits package is offered including, but not limited to, medical, dental, vision, 401k and matching, PTO, disability and life insurance, employee assistance, flexible spending or health savings account, and other additional voluntary benefits.
