The Registered Dietitian (RD) I is responsible for providing comprehensive nutrition care by assessing nutritional status, identifying nutrition-related needs, and developing individual nutrition care plans and interventions. The RD I will work under general supervision to deliver nutrition education and support using Patient-Centered Education (PCE) principles to support optimal health outcomes for TrueCare patients or WIC program participants.
This position requires to work at the Escondido and Ramona WIC locations. 


Duties & Responsibilities:
  1. Assesses nutritional status, identifies nutrition-related needs, and develops and implements individualized nutrition care plans and interventions for TrueCare patients or WIC program participants in accordance with the Nutrition Care Process, as established by the Academy of Nutrition and Dietetics and in compliance with TrueCare or CDPH/WIC policies and procedures.
  1. Conducts group or individual nutrition education and support using Patient-Centered Education (PCE) principles to support optimal health outcomes.
  2. Delivers in-service and outreach nutrition education for TrueCare staff, interdisciplinary care teams, and physicians ensuring alignment with clinical standards and evidence-based practice.
  3. Refers TrueCare patients or WIC program participants to other TrueCare services and community programs; documents referrals and follow-up in accordance with policies and procedures.
  4. Mentors TrueCare and WIC staff, dietitians, dietetic interns, and volunteers.
  5. Attends, completes, and actively participates in department meetings, team huddles, continuing education, and trainings, as assigned.
  6. Flexibility to work at assigned TrueCare or WIC locations based on department needs and/or goals.
  7. Ensure punctual, regular, and timely attendance in accordance with the TrueCare Employee Handbook; report to work on time and as scheduled and remain at work for the entire duration of each shift; and follow TrueCare policies for notifying the supervisor of and documenting all planned and unplanned absences.

In the Health Center
  1. Completes thorough nutrition and health assessments and interventions based on clinical assessments, medications, and overall health status.
  2. Provides Medical Nutrition Therapy (MNT) and ongoing nutrition education to patients and/or family members using motivational interviewing techniques to support behavior change; collaborates with patients and/or family members to establish at least one individualized, measurable SMART nutrition goal at the end of each counseling session.
  3. Delivers MNT nutrition education and ongoing support to patients enrolled in TrueCare’s TrueWellness programs and initiatives, promoting engagement and achievement of individualized health and wellness goals.
  4. Within Women’s Health Services, the RD I provides nutrition and lactation education, counseling, and support in alignment with CPSP guidelines.
  1. Collaborates with providers and the interdisciplinary care team to create and implement individualized patient-centered nutrition care plans, while serving as the nutrition subject matter expert offering evidence-based guidance, recommendations, and clinical support to enhance optimal patient care outcomes.
  2. Reviews and documents nutrition education and support services provided to TrueCare patients using the health center’s Electronic Health Record (EHR) system for collaboration of care, auditing, and billing purposes.
  3. Ensures all patient documentation in the Electronic Health Record is accurate, timely, and compliant with TrueCare policies, HIPAA regulations, and all applicable standards.

     In the WIC Department
  1. Accurately determines WIC program eligibility in accordance with policies and procedures using the WIC Web Information System Exchange (WIC WISE).
  2. Assesses individual nutritional status and identifies appropriate nutrition risks and interventions by reviewing biochemical (hemoglobin/hematocrit), anthropometric (length/height and weight), dietary, and health data in accordance with policies and procedures.
  3. Completes thorough nutrition and health assessments during certification and recertification appointments providing nutrition education and breastfeeding support using Participant Centered Education (PCE) principles and techniques; provides high-risk (HR) counseling and nutritional intervention reinforcing Medical Nutrition Therapy (MNT) instructions provided by the participant’s healthcare provider.
  4. Provides ongoing nutrition and breastfeeding education to WIC participants (via phone, video, or in person) based on their Nutrition Education Plan (NEP); accurately documents Care Plans, Nutrition Education Contacts (NECs), and pertinent comments/alerts in WIC WISE.
  5. Accurately schedules WIC appointments in accordance with department scheduling guidelines and in alignment with participants’ Nutrition Education Plans (NEPs).
  6. Reviews and processes medical formula requests in accordance with policies and procedures; collaborates with physicians and healthcare staff to resolve incomplete or inaccurate medical requests for coordinated participant care.
  7. Serves as a liaison between participants and healthcare providers to facilitate coordination of care and ensure continuity and quality service.
  8. Promotes and supports breastfeeding as the recommended method of infant feeding in alignment with CDPH/WIC, TrueCare, and Breastfeeding Friendly Health Center guidelines.
  9. Assists with the Electric Breast Pump Loan Program, including pump distribution, documentation, and cleaning.
  10. Utilizes TrueCare’s Electronic Medical Records (EMR) system and CDPH/WIC approved systems to accurately assess and manage WIC participant data in compliance with confidentiality and privacy policies and procedures.
  11. Supports the Clinic Manager in implementing WIC policies, guidelines, and procedures; assists with training staff and mentorship to ensure consistent application and compliance.
  12. Supports WIC Program Administration through caseload maintenance activities, participation in workgroups, and outreach efforts.
  13. Attends, completes, and actively participates in WIC Department Meetings, team huddles, continuing education, and trainings, as assigned.

Qualifications:
•Master’s degree from an accredited institution in nutrition, dietetics, or a related field. (Dietitians licensed prior to January 1, 2024 must hold a minimum of a bachelor’s degree in nutrition, dietetics, or a related field.)
•Current registration with the Commission on Dietetic Registration (CDR).
•Basic knowledge of conversational Spanish and willing to learn to counsel in Spanish.
•Strong typing and computer skills with proficiency in Microsoft Office applications (Outlook, Word, Excel, PowerPoint, and Teams).

Desired Qualifications:
 
•Knowledge of the WIC program or CPSP program guidelines.
•Bilingual in English and Spanish.

This position pays $39.12 to $58.67 on an hourly basis.
