Case Manager / Administrative Assistant
Responsibilities:
• Conduct intake meetings, complete initial assessments, and help create individualized service plans that reflect each client’s immediate needs and longer-term goals.
• Guide clients through available community resources related to housing, income, health care, transportation, and other essential services while offering timely support during urgent situations.
• Track client progress, document updates thoroughly, and adjust case plans in collaboration with clients and partner agencies when circumstances change.
• Assist clients with practical administrative needs such as completing forms, preparing written requests, and providing language support in English and Spanish when needed.
• Maintain accurate and well-organized paper and electronic case records, ensuring information is entered into required databases and files remain current.
• Support front office activity by responding to calls, assisting visitors, and helping manage day-to-day administrative tasks that keep services running smoothly.
• Prepare reports and case-related summaries, review service activity data, and contribute observations that help strengthen program effectiveness.
• Respond professionally to client concerns, collaborate with staff and volunteers, and participate in meetings, trainings, and case debriefs as required.
• Follow agency procedures for incident documentation and submit required reports promptly while taking on additional duties that support program operations.
• Represent the organization with professionalism and cultural sensitivity when engaging with clients, colleagues, and external partners.
Qualifications:
• Bachelor’s degree in Social Work, Psychology, or a closely related field required; Master’s in Social Work preferred.
• At least 2+ years of relevant experience in case management, intake coordination, or a similar role that combines direct client support with administrative responsibilities.
• Experience working with underserved or highly vulnerable populations, including individuals affected by homelessness, mental health concerns, substance use, trauma, or violence.
• Knowledge of community-based resources and the ability to coordinate services across multiple agencies and support systems.
• Bilingual communication skills in English and Spanish required
• Proficiency with Microsoft Office and comfort learning case management databases and virtual communication platforms such as Zoom.
• Strong written and verbal communication, documentation, conflict resolution, crisis response, and data management skills.
