Case Manager
Responsibilities:
- Conduct client assessments and develop service plans
- Coordinate referrals, resources, and follow-up services
- Maintain accurate case files and documentation
- Advocate for client needs with community partners
- Monitor progress and update care or support plans as needed
Qualifications:
Requirements:
- Bachelor’s degree in social work, human services, or related field preferred
- Experience in case management, social services, or community programs
- Strong interpersonal and crisis-management skills
- Knowledge of local resources and referral processes
- Ability to maintain confidentiality and accurate documentation
Compensation
