Performs advanced, specialized work to assist in the organization and implementation of various projects and programs. Work is performed under minimal supervision within established policies with latitude for independent judgment. Works on special projects as assigned. May substitute for supervisor during his/her absence.

Major/Essential Functions

Employees in this position are members of the TTUHSC SON Combest Interprofessional Collaborative Practice (IPCP) team. The employees will perform advanced, specialized work in community outreach, behavioral health and chronic disease management in vulnerable health populations. The major essential functions include:
  • Will establish close working relationships with patients and members of an interprofessional team to identify non-medical drivers of health and reduce health disparities.
  • Promote goal setting and strategic planning for the betterment of overall wellness.
  • Accepts referrals and provides case management for Children and Pregnant Women (CPW), and chronic disease management for IPCP.
  • Assesses healthcare needs to provide care coordination, tailored health educ and identifies community resources.
  • Serves as patient advocate and assists in identification and improvement of service delivery requiring expertise in critical thinking skills to plan and coordinate care.
  • Documentation of case management activities in the electronic health record.
  • Assist in the organization and implementation of various projects and programs.
  • Perform work under minimal supervision within established policies with latitude for independent judgment.
  • Works on special projects as assigned.


Occasional Duties

Occasional duties may include:
  • Development of training and presenting to community, patients, and family.
  • Participation in research related activities.
  • May substitute for supervisor during their absence.


Required Qualifications

High school plus six years experience in related field. Additional education may substitute for experience on a year for year basis.

Preferred Qualifications

  • Be a certified Community Health Worker (CHW) or currently enrolled and attending a CHW program that will be completed within six months of employment.
  • Experience with application of procedures within the CHW scope of practice.
  • Experience in dealing with behavioral health issues and/or navigating the behavioral health system.
  • Experience in developing relationships with individuals with serious mental illness and/or substance use disorder, and a demonstrated ability to connect with and support this population.
  • Experience with software such as, but not limited to, Word, Excel, PowerPoint, Outlook Calendar, and Electronic Medical Records.
  • Working knowledge of HIPAA and maintaining confidentiality.
  • Experience in data collection.
  • Experience in conducting community referrals for clients.
  • Community outreach experience.
  • Ability to communicate effectively in Spanish and English.
