Case Manager
Responsibilities:
• Oversee a high-volume portfolio of residents, providing individualized case management that promotes housing stability and improved quality of life.
• Meet with tenants regularly through outreach, intake discussions, ongoing assessments, and follow-up visits to identify needs and adjust support plans.
• Develop strong working relationships with residents and use proactive engagement strategies to address concerns before they threaten housing retention.
• Assist tenants with benefit enrollment, ongoing eligibility matters, and timely rent payment coordination.
• Help resolve unit-related concerns by partnering with property staff and participating in routine room inspections and pest control visits when needed.
• Connect residents with community resources such as healthcare, behavioral health, social services, and employment programs, and support successful follow-through on referrals.
• Foster a supportive residential environment by organizing tenant groups, community activities, and events in collaboration with colleagues and site teams.
• Maintain accurate, confidential case documentation, including file setup, updates, service records, and archival tasks in accordance with program standards.
• Respond appropriately to urgent tenant situations by using de-escalation techniques, coordinating support, and reporting suspected abuse or neglect through proper channels.
• Participate in scheduled meetings, recurring resident support activities, and site-based administrative efforts such as recertification and check distribution support.
Qualifications:
• Bachelor’s degree required, or at least 2 years of verified case management experience serving homeless or formerly homeless individuals in place of a degree.• Minimum of 1 year of experience working with diverse, low-income, homeless, or formerly homeless populations.
• Prior hands-on experience in case management, outreach, or closely related human services work.
• Familiarity with San Francisco resources and service networks that support homeless and formerly homeless adults.
• Ability to manage organized and compliant case files for a large resident caseload while maintaining attention to detail.
• Proficiency with Microsoft Office applications and confidence handling administrative tasks such as data entry, filing, and documentation.
• Strong written communication skills with the ability to complete paperwork accurately and prepare clear business correspondence.
• Demonstrated problem-solving ability, sound judgment, and discretion when handling confidential or sensitive tenant information.
Compensation
