Overview
Are you looking to make a meaningful impact by delivering high-quality, personalized support to members of the World Trade Center Health Program (WTCHP)? Join a highly skilled, interdisciplinary clinical team dedicated to supporting individuals through a holistic and compassionate approach to care. In this role, Care Coordinators work closely with RN Case Managers and Social Workers to help members maximize their health and overall well-being.
Remote Care Coordinator Responsibilities
- Work remotely to support members in locating, accessing, and utilizing healthcare services, community resources, and support programs.
- Assess member needs, determine acuity, and monitor progress; present cases in interdisciplinary team meetings as needed.
- Serve as a central point of contact to help members navigate care, reduce barriers, and ensure access to appropriate services.
- Collaborate closely with RN Case Managers, Social Workers, and other team members to coordinate care and support treatment goals.
- Maintain accurate and timely documentation of member interactions and services in compliance with organizational, state, and regulatory requirements.
- Deliver high-quality, member-centered service while meeting program standards and supporting overall care coordination objectives.
Care Coordinator Qualifications
- High School diploma required, associate's degree (or higher) in clinical or medical health administration or related field preferred
- 3+ years of experience in medical record review, triage, customer service, prior authorizations, medical claims and billing, assessing patient health and/or related experience
- Attention to detail, timetables, and commitment to completing tasks
- Experience with Microsoft Windows and computer savvy
- Must be well organized, efficient, and able to work independently and within a team
- Responsible for having reliable High-Speed Cable or Fiber Optic Internet service and an Internet Router in an established home office
- Must have Excellent Communication Skills via Phone, email, Text, Verbal, and Documentation Skills, and provide 24-hour follow-up to all communication
- U.S. Citizenship
- Must be able to successfully pass a preliminary credit and background check prior to hiring and training

Sedgwick Government Solutions provides a reasonable range of compensation for roles that may be hired in jurisdictions requiring pay transparency in job postings. Actual compensation is influenced by a wide range of factors including but not limited to skill set, level of experience, and cost of a specific location. For the jurisdiction noted in this job posting only, the range of starting pay for this role is ($75,000-$95,000).
