Position Description
Provides primary care as part of a health care team in an ambulatory setting in a particular discipline. Provides patient education and counseling as well as physical assessment. May participate in teaching programs for students and other student residents. Work is performed under usual clinic conditions and requires a high degree of contact with patients, families, and other clinic personnel and outside professionals. Some positions in the classification require travel to outreach clinics and home visits, and participation in educational efforts and special programs that occur outside of normal working hours.

Required Qualifications
Education: Graduated from an approved, accredited nursing program.
License/Certification: Current RN license in the State of Texas or have the ability to transfer your license to Texas. Certification as a Nurse Practitioner in the appropriate nursing specialty from an approved certification program as recognized by the Texas State Board of Nurse Examiners must be attained prior to the employment start date. Recognition by the Texas Board of Nurse Examiners as an Advance Practitioner.

Major/Essential Functions
- Travel to patients' homes (Mileage Reimbursement Available) and provide primary care as a part of a health care team.
- Provide care for geriatric patients and patients with mobility challenges.
- Provide patient education and counseling.
- Conduct patient health and physical assessments in their homes.
- Complete all training required by the institution and department by the deadlines identified.
- Complete patient charts and enter data into the Electronic Health Records (EHR), meeting deadlines identified by the department.
- Attend departmental meetings, such as the monthly provider meeting, in addition to other meetings as assigned.
Occasional Duties
- Attend conferences to complete CEUs (Continuing Education Units) as needed.
- May participate in teaching programs and serve as a preceptor for teaching programs for students as identified by the department and supervisor.
- Travel to meetings in different locations(Mileage Reimbursement Available).

- Experience in home health care.
